Parents of Indian Lake students should go ahead and complete their online forms at https://indianlake.esvportal.com. You can also find the Online Form link on the Indian Lake School webpage ils-k12.org under the Parents Tab. Laptops will be available during Open House Night to get this important information.
***Please be advised, ILMS and ILHS students will NOT be issued a Chromebook until their online forms are completed and the $30 tech fee is paid. If you paid this fee last year, you will not be charged again this year.
In addition, seniors must have documentation of the required meningococcal vaccine before being issued a Chromebook.
We MUST have this information to make sure you and your family are getting all-calls and updates from school. Please log in to your account at https://indianlake.esvportal.com (also under the Parent tab at ils-k12.org) and resubmit your information. Most of the info should carry over from years past. Even if you have no changes, you must log in and resubmit for each student so we know you've checked it. Please do this immediately. If you are the parent of a new student in the district, please call 937-686-8601 to get directions on getting registered and setting up an account.
If you have a student-athlete, please fill out the Athletic Forms after you update the Demographics and EMA forms. You will not receive hard copies of the athletic forms, you must fill them out online. Instructions are included here. Please fill these out as soon as possible, especially if your student's sport season is already underway.
***ALSO NEW*** The Free and Reduced Lunch Application online at https://www.lunchapplication.com. You can also find it with the Indian Lake online forms. The Free and Reduced Lunch Application must be completed by October 1. If not, students will be considered full-pay.
