In preparation for summer break, please be aware that all medication kept at school must be picked up by the last day of school. Students are not permitted to transport medications home. A parent or guardian signature is required for medication to be signed out of the clinic. Any medication remaining at school after the last student day will be properly disposed of. This includes all prescription and non-prescription medications, inhalers, seizure meds, Epinephrine pens, ect. 

Visit to find all care plans and medication administration forms to prepare for next school year. New forms are required yearly as they expire at the end of each school year. All medication MUST be stored in the original container with the label matching the signed doctor’s order.  Having the medication form completed during the summer will ensure that your child can receive medication on the first day of school.

Please note that Medication Administration Forms are also required for medications that students self-carry.  Ohio law only lists 3 self-carry medications for school which include inhalers, epinephrine auto injector and glucagon. ORC 3313.718 also states that in order for students to self-carry epinephrine auto injector, a second backup is to be received by the school. There is an area on the administration form for both the doctor and parent to sign consent for the student to self-carry these medications.
