Important Immunization Information for parents of students entering 7th and 12th grade!
Students entering the 7th grade are required to have at least one dose of the meningococcal immunization and one Tdap vaccine prior to school entry.  Students entering the 12th grade must have a second dose of the meningococcal vaccine, with a minimum interval of 8 weeks between the 2 shots, prior to entry.  If the first dose of meningococcal vaccine was administered after the 16th birthday, a second dose is not required. 
Logan County Health District Immunization Clinics are open every Monday from 12:00 - 1:00 p.m.; Tuesdays from 3:00 - 4:00 p.m. and every Thursday morning from 10:00 - 11:00 a.m.   An afternoon clinic is offered on the 1st Thursday of each month from 3 - 6 p.m.  Call the Health District at 937-651-6186 to schedule an appointment.  The Health District accepts Medicaid and some private health insurance.  Please contact your family physician's office or the Logan County Health District  now to beat the rush and assure your child is ready to enter school in the fall.  
It is the parents' responsibility to bring a paper to the school secretary showing the date the shot was given and the name of the doctor's office or Health Department where the student received the shot.
