Important Immunization Information for parents of students entering 7th and 12th grade!
Students entering the 7th grade are required to have at least one dose of the meningococcal immunization and one Tdap vaccine prior to school entry. 
Students entering the 12th grade must have a second dose of the meningococcal vaccine, with a minimum interval of 8 weeks between the 2 shots, prior to school starting.  
If the first dose of meningococcal vaccine was administered after the 16th birthday, a second dose is not required. 

Logan County Health District Immunization Clinics
are on hold for school children at this time because of the corona virus pandemic. However, you can call 937-651-6186 for an appointment to receive immunizations at the Health Department. The Health District accepts Medicaid and some private health insurance. 
As always you may also contact your family physician's office to receive the needed immunizations as well. 

It is the parent and students' responsibility to bring a paper to the school secretary showing the date the shot was given and the name of the doctor's office or Health Department where the student received the shot.
