This is the image for the news article titled Important Fall 2021 Athletic InformationThe Mandatory HS and MS Fall Sports Parent Meeting is Thursday, Aug.12, 6:30 pm, in the HS Auditorium. At least one parent must accompany his/her athlete to the meeting. This will be a brief gathering to cover OHSAA rules. 
This meeting is for parents of ILMS and/or ILHS students who are participating in a fall sport. It does not include club/youth soccer or marching band members. 

In addition, please complete the Online Athletic forms that are found at https://indianlake.esvportal.com as soon as possible. Be sure to Save and Submit when complete. If you have already turned in your physical form to the administration office, please disregard the sections of the online athletic forms where it asks you to upload your physical form. 
Finally, please fill out the HIPPA form online at

You can either print this off, fill it out and return the form to the administration office or fill it out electronically and email the completed form to mrhat@ils-k12.org. The HIPPA form and online athletic forms must be complete prior to your athlete's first fall contest. 

Finally, ILHS ($40) and ILMS ($20) Athletic Fees are also due prior to your athlete's first fall contest. Those can be paid in the administration office.
